Job summary

Continuum Therapy Partners seeks Physical Therapists to join a therapist-led team and support a growing national footprint. You will evaluate patients, develop and implement individualized treatment plans, and collaborate with multidisciplinary colleagues to optimize outcomes.

The role requires a Doctor of Physical Therapy (DPT), an active state license, and NPTE readiness, with preference for 1 year of PT experience.

Qualifications

• DPT degree required.
• Current, unencumbered state PT license with NPTE.
• 1 year of PT experience preferred.
• Evaluates and implements individualized treatment plans with other disciplines.
• Documents progress and adjusts care plans based on outcomes.

Responsibilities

• Evaluate patients and develop personalized treatment plans.
• Coordinate with other disciplines to ensure integrated care.
• Educate patients and families on home exercise programs.
• Document progress and adjust plans as outcomes change.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
