Ascension Alexian Outpatient Center in Schaumburg, IL is seeking a Physical Therapist to develop and implement patient-centered plans of care across neurologic and orthopedic conditions.

Develop, implement, and regularly reassess individualized, patient-centered plans of care in collaboration with patients and families.
Deliver evidence-based interventions across complex diagnoses, including neurological and orthopedic conditions.
Collaborate with the interdisciplinary team to coordinate care, optimize outcomes, and support safe, effective discharge planning.
Utilize innovative rehabilitation technologies and best practices to maximize functional recovery and independence.
Minimum weekend or evening and holiday requirement.
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Physical Therapy (PT) license credentialed from the Illinois Department of Financial and Professional Regulation prior to hire date or job transfer date required. A valid license pending letter will be accepted until license is issued.
