A List Therapy Group is actively seeking a Home Health Physical Therapist (PT) to provide patient-centered care in home settings across Southern California. This independent-contractor role offers flexible scheduling, control over service areas, and reliable referral opportunities, backed by a supportive clinical and administrative network.

You will evaluate and treat patients at their homes, coordinate with care teams, document digitally, and grow your caseload with guidance from our staffing

Qualifications

• Active California Physical Therapist (PT) license
• Valid driver's license and reliable transportation
• Strong communication and documentation skills
• Ability to provide services as an independent contractor

Responsibilities

• Perform physical therapy evaluations, reassessments, treatments, and discharges
• Develop and implement individualized treatment plans
• Provide patient and caregiver education
• Collaborate with the care coordination team
• Complete accurate and timely electronic documentation

What You'll Do

As a home health Physical Therapist, you'll help patients improve mobility, strength, balance, safety, and functional independence in the comfort of their homes.
