Job summary

A List Therapy Group is seeking a Home Health Physical Therapist (PT) to join a therapist‑owned network in Southern California. You will assess, treat, and discharge patients in home settings, build care plans, and educate patients and families while coordinating with a care team. Use your CA license and own transportation to reach assigned coverage areas.

The role offers per‑visit compensation, bonus opportunities, and flexible scheduling to fit your caseload and preferred cities and ZIP codes.

Qualifications

• Active California Physical Therapist license required.
• Valid driver's license and reliable transportation.

What You'll Do
