Job summary

A List Therapy Group is a therapist-owned home health therapy network in Southern California. We connect physical therapists with patient referrals across Los Angeles, Orange, Riverside, San Bernardino, San Diego, and Ventura counties.

Join to build your caseload with flexible schedules and strong administrative support. As a Home Health PT, you’ll evaluate and treat patients in their homes, using digital documentation and a collaborative care-coordination approach.

Qualifications

• Active California Physical Therapist (PT) license required.
• Valid driver’s license and reliable transportation.
• Strong communication and documentation skills.
• Ability to work as an independent contractor.

Responsibilities

• Perform physical therapy evaluations, reassessments, treatments, and discharges.
• Develop and implement individualized treatment plans.
• Provide patient and caregiver education.
• Collaborate with the care coordination team.
• Complete accurate and timely electronic documentation.

What You'll Do

As a home health Physical Therapist, you'll help patients improve mobility, strength, balance, safety, and functional independence in the comfort of their homes.

Responsibilities include:
