Qualifications

Responsibilities

• Perform physical therapy evaluations, reassessments, treatments, and discharges.
• Develop and implement individualized treatment plans.

Referral Opportunities

We have home health physical therapy referral opportunities throughout Los Angeles County, Orange County, Riverside County, San Bernardino County, San Diego and Ventura County.

Our network serves communities including Los Angeles, Beverly Hills, Santa Monica, Pasadena, Glendale, Burbank, Long Beach, Newport Beach, Irvine, Riverside, Ontario, Thousand Oaks, San Diego and surrounding areas.

What You'll Do

As a home health Physical Therapist, you'll help patients improve mobility, strength, balance, safety, and functional independence in the comfort of their homes.

Responsibilities include:

• Perform physical therapy evaluations, reassessments, treatments, and discharges

• Develop and implement individualized treatment plans

• Provide patient and caregiver education

• Collaborate with the care coordination team

• Complete accurate and timely electronic documentation

Requirements

• Active California Physical Therapist (PT) license

• Valid driver's license and reliable transportation

• Strong communication and documentation skills

• Ability to provide services as an independent contractor

Home health experience is welcome but not required. Our team can help therapists new to home health become familiar with referral coordination and documentation workflows.
