A List Therapy Group in Southern California is seeking a Licensed Physical Therapist to join our flexible home health team. You will visit patients in their homes across Los Angeles, Orange, Riverside, and surrounding counties, with referrals provided to build your caseload.

Qualifications

• Active California Physical Therapist (PT) license
• Valid driver's license and reliable transportation
• Independent contractor status

What You'll Do

As a home health Physical Therapist, you'll help patients improve mobility, strength, balance, safety, and functional independence in the comfort of their homes.

Responsibilities include:

• Perform physical therapy evaluations, reassessments, treatments, and discharges
• Develop and implement individualized treatment plans
• Provide patient and caregiver education
• Collaborate with the care coordination team
• Complete accurate and timely electronic documentation

Requirements

• Active California Physical Therapist (PT) license
• Valid driver's license and reliable transportation
• Strong communication and documentation skills
• Ability to provide services as an independent contractor

Home health experience is welcome but not required. Our team can help therapists new to home health become familiar with referral coordination and documentation workflows.
