A List Therapy Group is seeking a Home Health Physical Therapist (PT) in Fresno, CA. Earn $90–$120 per visit with a $500 sign-on bonus and a $250 referral bonus, plus flexible scheduling and coverage area options.

Independent contractor status allows you to select your caseload within our therapist-owned network. Join a network of 500+ home health agencies, enjoy digital documentation, and receive administrative support to build your caseload in the cities and ZIPs you prefer.

Formación

• Active California PT license is required.
• Valid driver's license and reliable transportation.
• Strong communication and documentation skills.
• Ability to provide services as an independent contractor.

What You'll Do

As a home health Physical Therapist, you'll help patients improve mobility, strength, balance, safety, and functional independence in the comfort of their homes.

Responsibilities include:

• Perform physical therapy evaluations, reassessments, treatments, and discharges
• Develop and implement individualized treatment plans
• Provide patient and caregiver education
• Collaborate with the care coordination team

Requirements

Home health experience is welcome but not required. Our team can help therapists new to home health become familiar with referral coordination and documentation workflows.
