Job summary

A List Therapy Group, a leading home health therapy network in Southern California, is seeking a Physical Therapist to provide home-based care. You will evaluate, treat, and discharge patients, and tailor plans to improve mobility and independence in the home setting.

We offer per-visit compensation ($90–$120), a $500 sign-on bonus, and a $250 referral bonus, plus flexible scheduling and the ability to select coverage areas. Join a therapist-owned network with strong referral opportunities.

Qualifications

• Active California Physical Therapist license.
• Valid driver's license and reliable transportation.
• Strong communication and documentation skills.
• Ability to work as an independent contractor.

Responsibilities

• Perform physical therapy evaluations, reassessments, treatments, and discharges.
• Develop and implement individualized treatment plans.
• Provide patient and caregiver education.
• Collaborate with the care coordination team.
• Complete accurate and timely electronic documentation.

What You'll Do

As a home health Physical Therapist, you'll help patients improve mobility, strength, balance, safety, and functional independence in the comfort of their homes.

Responsibilities include:

• Perform physical therapy evaluations, reassessments, treatments, and discharges
• Develop and implement individualized treatment plans
• Provide patient and caregiver education
• Collaborate with the care coordination team
