Job summary

A List Therapy Group is seeking a Home Health Physical Therapist (PT) to join our Southern California network. This role offers per-visit compensation and the flexibility to select service areas, with a strong emphasis on patient referrals and administrative support to help you succeed.

As a home health PT, you will evaluate and treat patients in their homes, develop individualized plans, educate patients and caregivers, and coordinate with the care team.

Qualifications

• Active California PT license required.
• Valid driver's license and reliable transportation.
• Strong communication and documentation skills.
• Ability to work as an independent contractor.

Responsibilities

• Perform PT evaluations, reassessments, treatments, and discharges.
• Develop and implement individualized treatment plans.
• Provide patient and caregiver education.
• Collaborate with the care coordination team.
• Complete accurate and timely electronic documentation.

What You'll Do

As a home health Physical Therapist, you'll help patients improve mobility, strength, balance, safety, and functional independence in the comfort of their homes.
