Job summary

Qualifications

- Current BLS/CPR certification.
- Minimum 1 year of PT experience, preferably in a SNF/geriatric setting.
- Ability to collaborate within an interdisciplinary team.
Responsibilities
- Evaluate, diagnose, and treat patients with physical impairments in a Skilled Nursing Facility setting.
- Develop and implement individualized treatment plans based on patient needs and clinical findings.
- Supervise and collaborate with Physical Therapist Assistants to ensure effective care delivery.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on mobility, safety, and home exercise programs.
- Maintain accurate and timely documentation in compliance with facility and regulatorystandards.
