Job summary

AED CONSTRUCTORES in San Jose, CA, seeks a skilled Physical Therapist to serve a geriatric population in a Skilled Nursing Facility. You will evaluate patients, develop individualized treatment plans, and guide recovery after surgery or illness to improve mobility and independence in daily living.

The role requires an active California PT license and current BLS/CPR certification, with at least 1 year of SNF or geriatric experience preferred.

Qualifications

• Active California Physical Therapist license.
• Current BLS/CPR certification.
• Minimum 1 year PT experience, SNF/geriatric setting preferred.

Responsibilities

• Evaluate, diagnose, and treat patients in SNF setting.
• Develop and implement individualized treatment plans based on patient needs.
• Supervise and collaborate with PT Assistants to ensure quality care.
• Monitor patient progress and adjust plans as needed.
• Educate patients and caregivers on mobility, safety, and home exercises.
• Maintain accurate documentation in line with facility standards.
