Job summary

AED CONSTRUCTORES is seeking a licensed Physical Therapist to join a Skilled Nursing Facility in Placerville, CA. The role focuses on evaluating patients, developing individualized plans, and guiding recovery for geriatric and post-acute populations.

Qualifications

• Active California PT license.
• Current BLS/CPR certification.
• Minimum of 1 year PT experience, preferably SNF/geriatric.
• Strong clinical reasoning and leadership skills.
• Ability to work collaboratively within an interdisciplinary team.

Responsibilities

• Evaluate, diagnose, and treat patients with physical impairments in a Skilled Nursing Facility setting.
• Develop and implement individualized treatment plans based on patient needs and clinical findings.
• Supervise and collaborate with Physical Therapist Assistants to ensure effective care delivery.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on mobility, safety, and home exercise programs.
• Maintain accurate and timely documentation in compliance with facility and regulatory standards.
