AED CONSTRUCTORES is seeking a skilled Physical Therapist to join a Skilled Nursing Facility in Santa Rosa, California. You will evaluate patients, develop individualized treatment plans, and guide them toward greater independence in a geriatric/post-acute setting.

The role requires an active California PT license, BLS/CPR, and at least one year of PT experience in SNF/geriatric care. You will work with an interdisciplinary team to deliver patient-centered rehab and ensure accurate documentation.

Qualifications

• Active state license as a Physical Therapist in California.
• Current BLS/CPR certification.
• Minimum of 1 year of recent PT experience, ideally in a SNF or geriatric setting.
• Strong clinical reasoning and leadership skills.
• Ability to work collaboratively within an interdisciplinary team.

Responsabilités

• Evaluate, diagnose, and treat patients with physical impairments in a Skilled Nursing Facility setting.
• Develop and implement individualized treatment plans based on patient needs and clinical findings.
• Supervise and collaborate with Physical Therapist Assistants to ensure effective care delivery.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on mobility, safety, and home exercise programs.
