Job summary

AED CONSTRUCTORES is seeking a skilled Physical Therapist to join a Skilled Nursing Facility in Woodland, California. The role supports geriatric and post-acute patients, helping them regain mobility and independence through personalized evaluations and treatment plans.

You will evaluate patients, develop plans, supervise PT Assistants, monitor progress, and educate patients and caregivers. California licensure and BLS/CPR are required, with at least one year of PT experience preferred in

Qualifications

• Active California Physical Therapist license.
• Current BLS/CPR certification.
• Minimum 1 year PT experience, ideally in SNF or geriatric setting.
• Strong clinical reasoning and leadership skills.
• Ability to work collaboratively within an interdisciplinary team.

Responsibilities

• Evaluate, diagnose, and treat patients with physical impairments in a Skilled Nursing Facility setting.
• Develop and implement individualized treatment plans based on patient needs.
• Supervise and collaborate with PT Assistants to ensure effective care delivery.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on mobility, safety, and home exercise programs.
• Maintain accurate and timely documentation in compliance with facility and regulatory standards.
