Job summary

AED CONSTRUCTORES in Stockton, California, is seeking a skilled Physical Therapist to join a Skilled Nursing Facility serving geriatric and post-acute patients. You will evaluate patients, develop individualized treatment plans, and guide recovery toward greater independence and quality of life.

The role requires an active California PT license, BLS/CPR certification, and at least one year of recent SNF or geriatric PT experience.

Qualifications

• Active state license as a Physical Therapist in California.
• Current BLS/CPR certification.
• Minimum of 1 year of recent PT experience in SNF or geriatric setting.
• Strong clinical reasoning and leadership skills.
• Ability to work collaboratively within an interdisciplinary team.

Responsibilities

• Evaluate, diagnose, and treat patients with physical impairments in a SNF setting.
• Develop and implement individualized treatment plans based on patient needs and findings.
• Supervise and collaborate with Physical Therapist Assistants to ensure effective care delivery.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on mobility, safety, and home exercise programs.
• Maintain accurate and timely documentation in compliance with facility standards.
