Job summary

AED CONSTRUCTORES is seeking a skilled Physical Therapist to join a Skilled Nursing Facility in Merced, CA. You will evaluate patients, create personalized treatment plans, and guide individuals toward greater independence and improved mobility.

Responsibilities include supervising PT assistants, monitoring progress, educating patients and caregivers, and maintaining compliant documentation. California licensure and BLS/CPR are required, with a preference for 1 year in SNF or geriatric settings.

Qualifications

• Active California state license to practice physical therapy.
• BLS/CPR certification required.
• Minimum 1 year of recent PT experience, ideally in SNF or geriatric settings.

Responsibilities

• Evaluate, diagnose, and treat patients with physical impairments in SNF.
• Develop and implement individualized treatment plans based on patient needs.
• Supervise and collaborate with PT Assistants to ensure quality care.
• Monitor progress and modify plans as needed.
• Educate patients and caregivers on mobility, safety, and home exercise programs.
• Maintain accurate documentation in line with facility and regulatory standards.
