Job summary

AED CONSTRUCTORES is seeking a licensed Physical Therapist to join a Skilled Nursing Facility in Fresno, California. This role focuses on geriatric and post-acute patients, helping recovery after surgery, illness, or injury and improving mobility and independence.

You will evaluate patients, develop individualized treatment plans, supervise assistants, monitor progress, and educate patients and caregivers on mobility and home exercises, while ensuring accurate documentation.

Qualifications

• Active California Physical Therapist license.
• Current BLS/CPR certification.
• Minimum 1 year PT experience preferred, SNF/geriatric setting.

Responsibilities

• Evaluate, diagnose, and treat patients in a Skilled Nursing Facility.
• Develop and implement individualized treatment plans based on patient needs.
• Supervise and collaborate with Physical Therapist Assistants to ensure effective care.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers on mobility, safety, and home exercise programs.
• Maintain accurate and timely documentation in compliance with facility and regulatory standards.
