Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is seeking a Physical Therapist (PT) for full-time or part-time engagement. A $10K relocation bonus is offered to support joining our rehab team in a long-term care facility.

The role includes comprehensive PT evaluations, treatment planning, documentation, and participation in interdisciplinary care to drive resident outcomes and quality of life.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD's orders.
• Document findings on the standardized evaluation form, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
• Document patient's daily participation and progress on weekly progress note accurately and timely.
• Document updated treatment plan in timely manner as indicated.
• Document patient's discharge and in-services on a discharge summary as indicated.
• Document any caregiver or discharge in-services on an in-service form as indicated.
• Complete any and all other documentation forms required by Facility.
• Document care plans and information needed for the MDS as deemed necessary by the facility.
