Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is looking for a Physical Therapist (PT) for full-time or part-time roles. You will evaluate patients, develop treatment plans, document progress, and coordinate with physicians and the rehab team to deliver quality care.

Qualifications

• Graduate of an accredited university with a degree in Physical Therapy (B.S., DPT, MA or MS) recognized by the APTA.
• Holds current license/registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD orders.
• Develop a comprehensive treatment plan with long and short-term goals, frequency and duration.
• Document daily progress and weekly notes; ensure all discharge summaries are completed.
• Coordinate with the interdisciplinary team and attend facility meetings as needed.
• Document care plans for MDS and complete required documentation.
