Hertford Rehabilitation And Healthcare Center in Hertford, NC is hiring an Occupational Therapist (Full Time, Part Time, or PRN) to join a rehab team in a long-term care facility.

Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy recognized by the AOTA.
• SNF experience preferred.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive OT evaluation based on MD orders.
• Document findings on the standardized evaluation form timely and accurately.
• Provide a comprehensive treatment plan with long and short-term goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, recertifications and discharges.
• Provide comprehensive treatment to patients using modalities and modify as indicated with precautions.
• Document daily participation and progress on progress notes timely.
• Document monthly updated treatment plans on recertification comprehensively and timely.
• Document patient discharge on a discharge summary as indicated.
