Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC seeks a licensed Occupational Therapist (FT/PT/PRN) to join a dedicated rehab team. The role requires OT evaluation, treatment planning, and documenting patient progress within a long-term care setting.

Qualifications

• Graduate with an accredited OT degree recognized by AOTA.
• SNF experience preferred.
• Holds current license and/or registration in the state.

Responsibilities

• Provide comprehensive OT evaluations based on physician orders.
• Develop and document treatment plans with long and short-term goals.
• Deliver comprehensive therapy using approved modalities and adjust plans as needed.
• Document evaluations, progress notes, discharge summaries and other required forms.
• Coordinate care with interdisciplinary team and participate in facility meetings.

Education

Bachelor/Master/Doctorate in Occupational Therapy (AOTA recognized)
