Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is hiring a Physical Therapist (PT) for full-time or part-time work. Relocation bonus of 10K is offered to qualified candidates.

The role includes comprehensive PT evaluation, treatment planning, and interdisciplinary collaboration to provide quality resident care. Qualifications require a degree in Physical Therapy recognized by the APTA and a current NC license/registration.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy that is recognized by the APTA.
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD's orders.
• Document findings on the standardized evaluation form, accurately and adhering to all guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify patient treatment as indicated while adhering to precautions.
• Document patient's daily participation and progress on weekly progress note accurately and timely.
• Complete any and all other documentation forms required by Facility.
• Co-sign notes as required.
• Maintain professional relationship with co-workers, facility staff, patients and patient families.
• Attend facility rehab meetings and participate in scheduling and interdisciplinary rounds.
