Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is seeking an Occupational Therapist to join a care-focused rehab team. The role supports comprehensive OT evaluations, treatment planning, and patient progress documentation across FT/PT/PRN schedules. Qualifications

• Graduate of an accredited university with a B.S., MA, or M.S. or Doctorate in Occupational Therapy.
• Licensed and/or registered in the state as applicable.
• SNF experience preferred.

Responsibilities

• Provide a comprehensive OT evaluation based on MD orders.
• Develop a treatment plan with long- and short-term goals and modalities.
• Obtain MD orders for evaluations, treatments and discharges.
• Document progress, plans, and discharge summaries in a timely manner.
• Supervise junior staff, students and techs as needed; co-sign notes.
• Participate in interdisciplinary team meetings and scheduling.
