Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is hiring a Physical Therapist (PT) for full-time or part-time hours. The facility offers a 10K relocation bonus, competitive wages, and a comprehensive benefits package.

Responsibilities include PT evaluations, treatment planning, documentation, and interdisciplinary collaboration. Qualified candidates hold a PT degree recognized by the APTA and a current North Carolina license/registration.

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable)

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD's orders.
• Document findings on the standardized evaluation form, adhering to guidelines.
• Provide a comprehensive treatment plan including long and short term goals, frequency, duration and treatment modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Provide comprehensive treatment to patients utilizing modalities and modify treatment as indicated.
• Document patient's daily participation and progress on weekly progress notes.
• Document updated treatment plan and discharge summaries as needed.
• Complete any other documentation forms required by the facility.
• Attend facility and rehab meetings as indicated by Supervisor/Regional Manager.
• Supervise junior staff or students as needed; co-sign notes as required.
• Maintain professional relationships with staff, patients, families.
• Provide in-service education to rehab staff, facility staff, hospitals, and community.
