Job summary

Hertford Rehabilitation And Healthcare Center in Hertford, NC is looking for a Physical Therapist (PT) to join our long-term care rehab team. This is a full-time or part-time role with MD orders-based evaluations and patient-centered treatment planning.

Qualifications

• Graduate of an accredited university with a DPT recognized by APTA.
• Holds current license/registration in North Carolina.

Responsibilities

• Provide a comprehensive PT evaluation based on MD orders.
• Develop a comprehensive treatment plan with goals, frequency, and modalities.
• Document daily participation, weekly progress, and discharge summaries.
• Ensure MD orders for evaluations, treatments, and re-certifications are obtained.
• Supervise junior staff or students as needed and co-sign notes.
