Responsibilities

• Provide comprehensive PT evaluation based on MD orders.
• Document findings on the evaluation form following guidelines.
• Develop and implement a comprehensive treatment plan with goals, frequency, and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Deliver treatment using modalities and adjust plan per patient progress.
• Document daily participation and weekly progress accurately and timely.
