Job summary

Qualifications

• Graduate of an accredited university with a B.S., Doctorate, MA or MS in Physical Therapy recognized by the APTA.
• Holds current license and/or registration in the state (as applicable).

Responsibilities

• Provide a comprehensive physical therapy evaluation based on MD orders.
• Develop a comprehensive treatment plan with long and short-term goals, frequency, duration and modalities.
• Ensure MD orders are obtained for evaluations, treatments, re-certifications and discharges.
• Deliver comprehensive treatment using modalities and modify plan as indicated, following precautions.
• Document daily participation and progress on weekly progress notes and update treatment plans.
• Document discharge and in-services on a discharge summary and any in-service forms.
• Complete required facility documentation and care plans for MDS as needed.
• Attend facility rehab meetings and participate in interdisciplinary team activities.
• Supervise junior staff or students and co-sign notes as required.
• Maintain professional relations with staff, patients and families; provide in-service education as needed.
• Adhere to HR policies and maintain a positive work environment.

Education
