Job summary

Continuum Therapy Partners, LLC seeks a Physical Therapist to evaluate and treat patients at the Dade City Health and Rehabilitation Center in Dade City, FL. The role requires a Doctor of Physical Therapy (DPT), an active state license, and NPTE completion.

Prior PT experience is preferred, with opportunities to collaborate across disciplines. The position is PRN with daytime shifts, based at the Dade City location.

Qualifications

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
