Job summary

Continuum Therapy Partners, LLC is seeking a licensed Physical Therapist to join our growing Florida network. The role focuses on evaluating patients, developing individualized care plans, and coordinating with a multidisciplinary team to improve quality of life at Longwood Health and Rehabilitation Center in Longwood, FL.

The ideal candidate holds a DPT, an active state license, and NPTE passage. At least one year of experience is preferred, with PRN daytime shifts available.

Responsibilities

• Evaluates and treats patients using individualized plans of care.

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
• Educates patients and families on home exercise programs and injury prevention
