Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our team at Elroy Health Services. The candidate will evaluate, develop, and implement individualized treatment plans and educate patients on exercises and injury prevention.

Requirements include a Doctor of Physical Therapy (DPT), an active state license, and NPTE eligibility. Prior PT experience is preferred; PRN scheduling applies to this position.

Qualifications

• DPT degree required.
• Current, unencumbered state PT license; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.

Physical Therapist Requirements & Responsibilities
