Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our clinician-led network in Ohio. This role focuses on delivering individualized treatment plans and advancing patient outcomes across a growing practice.

The ideal candidate holds a Doctor of Physical Therapy (DPT), an active state license, and prefers at least one year of experience. You will collaborate with a multidisciplinary team and educate patients on home programs to prevent re-injury.

Qualifications

• DPT degree required.
• Current, unencumbered state Physical Therapy License; NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Educates patients and families on home exercise programs and injury prevention.

Responsibilities

• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
