Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist to join our Omaha, NE team. A Doctor of Physical Therapy (DPT) and an active state license (NPTE) are required, with 1 year of PT experience preferred.

You will evaluate, develop, and implement individualized treatment plans in coordination with other disciplines; document patient progress and adjust plans based on outcomes; and educate patients and families on home exercise programs and injury prevention.

Qualifications

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on clinical outcomes
