Job summary

Continuum Therapy Partners seeks a dedicated Physical Therapist Assistant to support patient care at Montgomery Nursing & Rehab Center in Hillsboro, IL. The role involves implementing treatment plans under a Physical Therapist, tracking progress, and educating patients on home programs and device use.

Part-time daytime hours are available, with a focus on delivering high-quality, therapist-centered care. Ideal candidates have an Associate Degree in PTA and IL PTA licensure, plus preferred PTA

• Associate degree in Physical Therapist Assistant required.
• Current, unencumbered Illinois PTA license required.
• One year of PTA experience preferred.

Responsibilities

• Implements individualized treatment plans under supervision of a PT.
• Documents patient progress and reports changes in status to supervising PT.
• Educates patients on home exercise programs and proper use of assistive devices.

Physical Therapist Assistant Requirements & Responsibilities:

• Associate Degree in Physical Therapist Assistant Program required
• Current, unencumbered state Physical Therapist Assistant License
• 1 year of previous experience as a Physical Therapist Assistant preferred
• Implements individualized treatment plans under the supervision of a Physical Therapist
• Documents patient progress and reports changes in patient status to the supervising PT
• Educates patients on home exercise programs and proper use of assistive devices
