Qualifications

• Current or eligible Physical Therapist Assistant license in the state of practice.
• Ability to collaborate with supervising PT and care teams.
• Familiarity with documentation standards and regulatory requirements.

Responsibilities

• Treat residents with disorders or deficits per physician orders and evaluations.
• Optimize functional abilities and skills of patients.
• Document all activities in Net Health and maintain accurate records.
• Collaborate with supervising PT to develop resident-specific programs.
• Attend care planning, staff and family meetings as required.
• Assist in meeting OBRA and certification standards while following guidelines.
• Follow facility infection control policies and procedures.
• Perform other duties as assigned by supervisor.

Key Responsibilities

• Responsible for the treatment of residents with noted disorders or deficits within Physical Therapy scope of practice upon receiving physician order and completion of evaluation

• Optimize the functionality and skills of patients

• Completes all necessary documentation via Net Health

• Collaborate with supervising PT developing programs appropriate to the needs of facility residents while fulfilling PTA/PT co-signature and supervision requirements

• Attend all required meetings including care planning, staff meetings, family meetings, etc.

• Assist the facility in meeting the standards necessary to obtain state and federal certification while following OBRA Resident Right Guidelines

• Follow facility infection control policies and procedures

• Complete all other tasks that are deemed appropriate for the role and assigned by the manager/supervisor

Qualifications
