Family Home Health Network is seeking a Physical Therapist to deliver PT services to patients in the Chicagoland area. You will assess and evaluate functional status and participate in developing the total plan of care, focusing on muscle strength, mobility, and gait.

You will instruct patients and caregivers in the use of therapeutic appliances and determine priority needs for physical therapy, with emphasis on delivering high-quality, patient-centered care.

Formación

• Il license as PT required.
• CPR card required.
• Valid IL driver’s license with auto insurance.
• Home Care Home Base experience preferred.

Responsabilidades

• Provide physical therapy services including assessment, evaluation and plan of care development.
• Assess muscle strength, mobility, and gait.
• Instruct patients, families and caregivers in use of therapeutic appliances and determine priority PT needs.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:

Baccalaureate or master’s program in physical therapy approved by an accredited organization

• Current Illinois license as a Physical Therapist
• Current CPR card
• Valid IL driver’s license and auto insurance policy
• Home Care Home Base experience preferred
