Job summary

Family Home Health Network is seeking a Physical Therapist to deliver home-based care across the Chicagoland area. You will evaluate patients, develop and contribute to the plan of care, and support rehabilitation goals in a home setting.

The role requires an Illinois license, CPR certification, and a valid IL driver's license with auto insurance. Home Care Home Base experience is preferred, and schedules are flexible to meet patient and family needs.

Qualifications

• Baccalaureate or master's program in physical therapy approved by an accredited organization.
• Current Illinois license as a Physical Therapist.
• Current CPR card.
• Valid IL driver's license and auto insurance policy.

Responsibilities

• Provide physical therapy services, assess and evaluate therapeutic/rehabilitative/functional status, and contribute to the total plan of care.
• Assess muscle strength, mobility, gait; instruct patients and caregivers in use of therapeutic appliances.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.
