Family Home Health Network is seeking a Physical Therapist to deliver home-based services across the Chicagoland area. You will assess patients, develop and adjust the plan of care, and address muscle strength, mobility, and gait as part of rehabilitation.

Responsibilities include patient and caregiver instruction on exercises and devices, ensuring safety, and coordinating with families to prioritize therapy needs. A flexible schedule is emphasized.

Qualifications

• Must have a valid IL Physical Therapy license.
• Baccalaureate or master’s in physical therapy from an accredited program.
• Current CPR certification.

Responsabilités

• Provide PT services for home health patients including assessment and plan of care.
• Instruct patients and caregivers in use of therapeutic devices and exercises.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:

• Baccalaureate or master’s program in physical therapy approved by an accredited organization

• Current Illinois license as a Physical Therapist

• Current CPR card

• Valid IL driver’s license and auto insurance policy

• Home Care Home Base experience preferred
