Job summary

Family Home Health Network is seeking a Physical Therapist to deliver home health services across the Chicagoland area. The role includes assessing patients, evaluating functional status, and contributing to the overall care plan.

The position requires a PT degree, Illinois license, CPR cert, and a valid IL driver’s license. Home Care Home Base experience is preferred. A comprehensive benefits package is provided.

Qualifications

• Baccalaureate or master’s degree in physical therapy (accredited)
• Current Illinois PT license
• CPR certification is current
• Valid IL driver’s license and auto insurance
• Home Care Home Base experience preferred

Responsibilities

• Provide physical therapy services and assess/ evaluate therapeutic status and plan of care.
• Participate in development of the total plan of care.
• Assess muscle strength, mobility, and gait.
• Instruct patients, families, and caregivers in use and care of therapeutic appliances.
• Determine priority needs for physical therapy services.

Job description

Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:

• Baccalaureate or master's program in physical therapy approved by an accredited organization
• Current Illinois license as a Physical Therapist
• Current CPR card
• Valid IL driver's license and auto insurance policy
• Home Care Home Base experience preferred
