Job summary

Family Home Health Network is seeking a Physical Therapist to deliver home health services across the Chicagoland area. The role involves assessing therapeutic status and contributing to the overall plan of care, including mobility and gait analysis.

You will instruct patients and families on therapeutic devices and determine priorities for therapy, with emphasis on maintaining patient independence in daily activities.

Qualifications

• Baccalaureate or master's program in physical therapy approved by an accredited organization.
• Current Illinois license as a Physical Therapist.
• Current CPR card.
• Valid IL driver's license and auto insurance policy.
• Home Care Home Base experience preferred.

Responsibilities

• Provide physical therapy services including assessment and evaluation of therapeutic/rehabilitative/functional status and participation in the total plan of care.
• Assess muscle strength, mobility, and gait; determine priority needs for physical therapy.
• Instruct patients, families and caregivers in the use and care of therapeutic appliances.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.
