Job summary

Family Home Health Network in the Chicagoland area is seeking a licensed Physical Therapist to provide PT services to patients in home health settings.

You will assess and evaluate therapeutic/rehabilitative/functional status and participate in developing the total plan of care; assess muscle strength, mobility, and gait; and instruct patients, families, and caregivers in the use and care of therapeutic appliances.

Qualifications

• Master’s program in physical therapy approved by an accredited organization.
• Current Illinois license as a Physical Therapist.
• Current CPR card.
• Valid IL driver’s license and auto insurance policy.
• Home Care Home Base experience preferred.

Responsibilities

• Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care.
• Assess muscle strength, mobility, and gait.
• Instruct patients, families and caregivers in the use and care of therapeutic appliances.

Job description

Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.
