Family Home Health Network in Illinois is seeking a Physical Therapist to provide home health services, assess functional status, and participate in developing the total plan of care for patients in the Chicagoland area.

You will evaluate muscle strength, mobility, gait, educate patients and caregivers on exercises and use of therapeutic devices, and help set priorities for care based on patient needs. A strong team ethic and patient-centered approach are essential.

Qualifications

• Illinois license to practice physical therapy.
• CPR certification.
• Valid IL driver’s license and auto insurance.
• Home Care Home Base experience preferred.

Responsabilités

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:

Baccalaureate or master's program in physical therapy approved by an accredited organization

• Current Illinois license as a Physical Therapist
• Current CPR card
• Valid IL driver's license and auto insurance policy
• Home Care Home Base experience preferred
