Job summary

Family Home Health Network in the Chicagoland area is seeking a Physical Therapist to deliver in-home physical therapy services, assess functional status, and contribute to the total plan of care. You will educate patients and caregivers on therapies and therapeutic devices to support mobility and independence.

Qualifications include a Baccalaureate or master’s degree in physical therapy, an active IL license, CPR, and a valid IL driver’s license with auto insurance.

Qualifications

• Baccalaureate or master's degree in physical therapy from an accredited program.
• Current Illinois license as a Physical Therapist.
• Current CPR certification.
• Valid IL driver’s license and auto insurance.
• Home Care Home Base experience preferred.

Responsibilities

• Provide physical therapy services to home health patients, including assessment of therapeutic/functional status.
• Develop, implement, and adjust the total plan of care with the patient and care team.
• Instruct patients, families and caregivers in therapy techniques and device care.

Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications

Baccalaureate or master's program in physical therapy approved by an accredited organization

• Current Illinois license as a Physical Therapist
• Current CPR card
• Valid IL driver's license and auto insurance policy
• Home Care Home Base experience preferred
