Job summary

Family Home Health Network in the Chicagoland area provides intermittent home health, palliative and hospice care. We are seeking a Physical Therapist to join our team.

You will assess therapeutic status, participate in developing the total plan of care, evaluate strength, mobility and gait, and educate patients, families and caregivers in use of therapeutic appliances.

Qualifications

• Active Illinois license to practice physical therapy.
• Current CPR certification.
• Valid IL driver’s license and auto insurance.
• Home Care Home Base experience preferred.

Responsibilities

• Provide physical therapy services to patients in home health, palliative and hospice settings.
• Assess therapeutic/rehabilitative status and assist in developing the plan of care.
• Evaluate muscle strength, mobility and gait, and determine priority PT needs.
• Instruct patients, families and caregivers in use and care of therapeutic appliances.

Family Home Health Network, is a provider of intermittent Home Health care, palliative care, hospice care and custodial private care services throughout the Chicagoland area.

The Physical Therapist is responsible for providing physical therapy services to patients including but will not be limited to assessing and evaluating therapeutic/rehabilitative/functional status and participating in the development of the total plan of care; assessing for muscle strength, mobility, gait. The Physical Therapist will also instruct patients, families and caregivers in the use and care of therapeutic appliances and determine priority needs for physical therapy.

Qualifications:

Baccalaureate or master's program in physical therapy approved by an accredited organization

• Current Illinois license as a Physical Therapist
• Current CPR card
• Valid IL driver's license and auto insurance policy
• Home Care Home Base experience preferred
