Job summary

CARIN Alliance is seeking a licensed physical therapist to provide in-home and senior living care under the Geriatric House Calls™ model. The role focuses on one-on-one patient care, evaluations, and tailored treatment plans across private homes and communities.

You will document thoroughly in EMR systems, collaborate with interdisciplinary teams, and leverage telehealth to improve access and continuity of care for older adults.

Qualifications

• Valid PT license or eligibility in practice state(s).
• Degree from an accredited PT program.
• Proficiency in computer use and EMR documentation.
• Current CPR certification.
