Job summary

Provident Behavioral Health is seeking a Physical Therapist to provide personalized care for older adults in private homes and senior living communities in Florida. Services include in-home and telehealth therapy, documentation in EMR, and collaboration with an interdisciplinary team to support mobility and independence.

The position emphasizes aging-in-place education for patients and caregivers, PPE adherence, and flexible scheduling across full-time, part-time, and PRN roles.

Qualifications

• Degree from an accredited Physical Therapy program.
• Proficiency with EMR and basic computer skills.
• Current CPR certification.

Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
• Provide in-home and virtual therapy sessions, documenting accurately within the EMR system.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth to enhance care accessibility and flexibility.

Responsibilities

• Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility, independence, and wellness.
• Provide in-home and virtual therapy sessions, documenting accurately within the EMR system.
• Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff on safety and aging-in-place strategies.
• Utilize telehealth to enhance care accessibility and flexibility.

Qualifications & Skills

• Valid state license or eligibility for licensure in Physical Therapy.
• Degree from an accredited program and proficiency with EMR and basic computer skills.
• Current CPR certification.

Physical & Other Requirements: Ability to perform moderate physical effort, assist with patient transfers, and drive to various locations. Must meet physical demands of community-based care and adhere to PPE protocols.
