Job summary

Health Here is seeking a licensed Physical Therapist to deliver one-on-one care to older adults in private residences and senior living communities as part of the Geriatric House Calls model. The role offers autonomy, flexible scheduling, and both in-person and telehealth modalities.

You will evaluate, treat, document in EMR, and collaborate with an interdisciplinary team while educating patients and caregivers to promote safe mobility and aging in place.

Qualifications

• Active Physical Therapy license or eligibility in the practice state.
• Graduate of an accredited Physical Therapy program.
• Proficiency with computers and EMR systems; CPR certified.

Responsibilities

• Provide individualized physical therapy to seniors in their homes or community settings
• Conduct evaluations, develop treatment plans, and administer evidence-based therapies to enhance mobility, independence, and wellness
• Maintain accurate documentation within electronic medical records
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Utilize telehealth options to extend care access and flexibility

This position offers autonomy, flexibility, and the opportunity to build meaningful patient relationships, incorporating in-person and virtual care modalities.

Key Responsibilities

• Provide individualized physical therapy to seniors in their homes or community settings
• Conduct evaluations, develop treatment plans, and administer evidence-based therapies to enhance mobility, independence, and wellness
• Maintain accurate documentation within electronic medical records
• Collaborate with interdisciplinary teams including PTs, OTs, SLPs, and Fitness Specialists

• Utilize telehealth options to extend care access and flexibility

Qualifications & Skills

• Valid Physical Therapy license or eligibility in the state of practice
• Degree from an accredited Physical Therapy program
• Proficiency with computers and EMR systems
• Current CPR certification

Physical Requirements

Ability to perform moderate physical activities including lifting, repositioning patients, assisting with transfers, and extended walking, as well as driving to patient locations when necessary.
