Job summary

Consumer Directed Services Inc. seeks a Licensed Physical Therapist to provide in-home and senior community care under the Geriatric House Calls™ model. The role emphasizes autonomy, collaboration with clinicians, and patient-centered outcomes.

Responsibilities include evaluations, treatment planning, documentation, and telehealth-enabled care to enhance access and continuity for older adults. Options for full-time, part-time, or PRN arrangements are available.

Qualifications

• Valid physical therapy license and degree from an accredited program.
• Proficiency in computer systems and EMR software.
• Current CPR certification.
• Ability to perform physical tasks including lifting, repositioning, and assisting with transfers.
