Job summary

Embassy of Huntingdon Park in Huntingdon Park, PA, seeks a compassionate Physical Therapist (PT) for part-time/PRN work within our in-house rehabilitation team. The role focuses on improving mobility, strength, balance, and independence for residents through personalized plans.

Qualifications

• Active Physical Therapy license in PA in good standing.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of the Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled therapy to reduce pain, restore function, and promote healing.
• Monitor resident progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Ensure services meet all professional, legal, and regulatory standards.
