Job summary

Embassy of Woodland Park in Orbisonia, PA is seeking a compassionate Physical Therapist (PT) to join its in-house rehabilitation team on a part-time/PRN basis. New graduates are welcome to apply and will provide resident-centered therapy to improve mobility, strength, balance, and independence.

You will develop individualized treatment plans, educate residents and families, and collaborate with an interdisciplinary team to ensure high-quality care and compliance with regulatory standards.

Qualifications

• Active Physical Therapy license in PA in good standing.
• Experience in skilled nursing or long-term care preferred.
• Strong clinical, communication, and organizational skills.
• Familiarity with Net Health EDS preferred.

Responsibilities

• Evaluate residents' mobility, strength, balance, and functional needs.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled therapy to reduce pain, restore function and promote healing.
• Monitor resident progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Ensure services meet all professional, legal, and regulatory standards.
